WHAT TO EXPECT
On the day of your first visit to our office, we recommend arriving a few minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office.
WHAT TO BRING TO YOUR FIRST VISIT
If you were unable to complete our paperwork prior to your visit, please be prepared to answer questions regarding you and your family’s medical history. To help expedite the first appointment, we ask that you please bring your:
- Insurance information
- Medical records
- (HMO patients): proper authorization or referral forms
INSURANCE AND BILLING
We accept most PPO’s and HMO’s through HealthCare Partners. Please call or email us for information specific to your insurance company and plan. If you are a patient of Dr. Gorlick’s, please contact Argus Medical Management with any billing questions. For all other billing inquiries, please contact Stephanie Mason.
We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own physician.
